新型冠状病毒感染疫情对湖南省结核病预防控制机构工作的影响研究

Study on the influence of COVID-19 epidemic on the work of tuberculosis prevention and control institutions in Hunan Province

摘要:

目的: 了解2020年湖南省新型冠状病毒感染(简称“新冠感染”)疫情不同应急响应级别对湖南省结核病防治(简称“结防”)机构人力资源调用及常规工作的影响。方法: 通过对省、市州、县市区三级结防机构进行问卷调查,比较新冠感染疫情防控不同级别响应期间,各级结防机构结核病防治人力资源和结核病防控工作状况。同时,对比分析新冠感染疫情前和疫情期间(2019年和2020年)结防机构开展的常规工作(包括举办工作会议、培训和督导)发生的变化。共收回140份有效问卷,占全省结防机构的96.6%(140/145)。 结果: 2020年,140家机构中有134家(95.7%,134/140)结防人员曾经被借调。按借调人数算,共358名(87.1%,358/411)人员曾经被借调;按借调人员工作时长算,共29.2%(1281.5/4385.7)的工作时长被借用。2020年全省召开工作会议、培训和督导次数分别为105、181和528次,较2019年分别减少了30.0%(45/150)、24.3%(58/239)和28.1%(206/734)。从省、市、县三级分层分析结果来看,县市区级结防机构各项活动减少的频次以应急响应期更加明显,2019年1—3月会议活动、培训活动和督导活动分别占全年的36.9%(48/130)、25.7%(56/218)、20.2%(133/659),2020年1—3月分别占11.3%(11/97)、9.0%(15/166)、10.9%(52/478),差异均有统计学意义(χ2值分别为18.901、17.339、17.602,P值均<0.001)。 结论: 新冠感染疫情对湖南省结防机构工作带来了冲击与挑战,其中对应急响应期影响较大。未来应加强结核病防治各项工作措施,以尽快消除新冠感染疫情带来的影响。

Abstract:

Objective: To understand the impact of different levels of emergency response to COVID-19 in 2020 on human resource and routine work of tuberculosis (TB) prevention and control institutions in Hunan Province. Methods: A questionnaire survey was conducted among the TB prevention and control institutions at provincial, municipal and county levels to compare the human resource and the work status of TB prevention and control institutions during the different levels of response for COVID-19. Routine work conducted by the prevention institutions before and after the COVID-19 outbreak (2019 and 2020), including working meetings, training and supervision activities were compared. A total of 140 pieces of valid questionnaire were retrieved, accounting for 96.6% (140/145) of all TB prevention and control institutions in the whole province. Results: In 2020, 134 of the 140 institutions (95.7%, 134/140) had staff being temporarily reallocated. Three hundred and fifty-eight (87.1%, 358/411) of staff and 29.2% (1281.5/4385.7) of these staff’s working time had been reallocated to COVID-19 response. In 2020, the number of work meetings, training and supervision held in the province were 105, 181 and 528, respectively, which decreased by 30.0% (45/150), 24.3% (58/239) and 28.1% (206/734) compared with 2019. According to the analysis results at the provincial, municipal and county levels, the reduction of working activities of county level was more obvious in the emergency response period (January-March 2020). From January to March 2019, meeting activities, training activities, and supervision activities accounted for 36.9% (48/130), 25.7% (56/218), and 20.2% (133/659) of the entire year, respectively, while from January to March 2020, they accounted for only 11.3% (11/97), 9.0% (15/166), and 10.9% (52/478) of the entire year of 2020, and the differences were statistically significant (χ2=18.901, 17.339, 17.602; all P<0.001). Conclusion: The COVID-19 epidemic has impacts on and brought challenges to the work of TB prevention and control institutions in Hunan Province, especially during the emergency response period. Measures for TB prevention and control should be strengthened to eliminate the impact of COVID-19 as soon as possible.
