慢性阻塞性肺疾病生物制剂治疗进展及临床应用——基于2026 GOLD指南

摘要：

慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)全球疾病负担沉重,常规治疗方案对高2型炎症表型难治性患者的病情控制效果欠佳。生物制剂的问世为该类患者提供了全新靶向治疗路径,2026版慢性阻塞性肺疾病全球倡议(Global Initiative for Chronic Obstructive Lung Disease,GOLD)指南已将其纳入COPD规范化诊疗体系。笔者围绕近3年COPD相关生物制剂的作用机制、关键临床研究、指南推荐要点及临床实践规范展开综述,结合我国人群用药特点与中西医结合研究现状,剖析当前临床应用的现存问题与未来发展方向,旨在为临床精准选择与合理使用生物制剂提供参考。

Abstract:

Chronic obstructive pulmonary disease (COPD) imposes a heavy global disease burden. Conventional therapeutic regimens achieve unsatisfactory disease control in refractory patients with high type 2 inflammation phenotypes. The advent of biologics has provided a novel targeted therapeutic approach for such patients, and the 2026 Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines have incorporated biologics into the standardized diagnosis and treatment system of COPD. This paper reviews the mechanism key clinical studies, guideline recommendations and clinical practice specifications of COPD-related biologics in the past three years. Combined with the medication characteristics of the Chinese population and the research status of integrated traditional Chinese and Western medicine, it analyzes the existing problems in current clinical application and future development directions, so as to provide a reference for the accurate clinical selection and rational use of biologics.
