Abstract:

The Chinese Society for Tuberculosis of the Chinese Medical Association and the Department of Occupational Health and Occupational Diseases of the Chinese Preventive Medicine Association organized multidisciplinary experts in respiratory diseases, occupational diseases, tuberculosis and other relevant fields to jointly formulate the Chinese Expert Consensus on Diagnosis and Treatment of Pneumoconiosis Complicated with Pulmonary Tuberculosis. The consensus establishes integrated diagnostic principles for pneumoconiosis complicated with pulmonary tuberculosis, and specifies pathological classification, judgment of active imaging-based signs, and application specifications for laboratory and invasive examinations. Stratified differential diagnostic criteria for non-tuberculous mycobacterial pulmonary disease and pulmonary aspergillosis have been formulated. Clinical pathways including anti-tuberculosis treatment regimens and duration, management of drug-drug interactions, surgical and interventional therapy, rehabilitation and psychological intervention are standardized. Meanwhile, it defines applicable indications for end-stage life support, lung transplant evaluation and palliative care, and introduces international prevention-and-control experience regarding two-way screening and joint management. Based on the 12 core clinical questions and 13 recommendations proposed in the consensus, the authors systematically interpret the core contents and evidence-based rationale of this consensus. Combined with supplementary domestic and international research evidence on epidemiology, diagnostic techniques, therapeutic regimens and prevention-control models, this paper summarizes the key points of five modules: diagnostic strategies, differential diagnosis, treatment and management, end-stage management, and prevention-control systems.
